To schedule an appointment with Pediatric Urology Associates please contact our main office at 732-613-9144. We ask that if you wish to be seen at our satellite office, that you notify our scheduler when you call. At the time that you make your appointment please confirm with the scheduler: the physician your child will be seeing; the office location; time of the appointment; your insurance and referral requirements. Referrals if required, are the patient’s responsibility and should be obtained in advance from your primary care physician (PCP) prior to your child’s appointment. If you arrive without a referral, we will not contact your PCP on the day of the visit and you will be asked to reschedule your appointment.

Some patients will be asked to come with a full bladder so that we can perform in office testing. If your child does not come with a full bladder you will be asked to reschedule your child’s appointment.

We recognize your time is valuable. However, from time to time there may be unavoidable delays due to high patient volume or an emergency situation. We appreciate your patience and will do our best to see your child in a timely fashion. If the delay is problematic, you can reschedule if necessary.

We require at least 24 hour notice for appointment cancellations or changes. Same day cancellations are subject to service charges. If you arrive more than 20 minutes late for your appointment you may be asked to re-schedule.

All new patients are asked to arrive at least 20 minutes in advance of their appointment to allow time to fill out necessary paperwork. New patients can down-load our patient registration form and complete it prior to the office visit.

Check list for new patients

  • Any imaging studies (x-ray, ultrasounds, CT scans, MRI or IVP) and/or diagnostic studies (i.e. blood tests or urine tests)
  • Current copy of your insurance card
  • Identification of the parent or guardian; if you are not the parent or guardian a letter from the parent or guardian stating you are permitted to bring the child to the office visit
  • Referral if this is required by your insurance provider
  • Method of payment : cash, check or Visa or Mastercard
  • Completed registration form
